Laminated VIS QR Code Tables
Vaccine Information Statements (VISs) explain both risks and benefits of vaccination to the recipient. If you give vaccinations, federal law requires you to document provision of the current federal VIS to each recipient before administering any vaccine covered by the federal Vaccine Injury Compensation Program. An easy, paperless way to comply with the law is to scan a QR code and access the current VIS from a smartphone or tablet.
Immunize.org now offers a laminated table of QR codes for commercially available vaccines (including vaccines for adults and children). Two tables are available: one has links to the official VIS (produced in English by CDC) and the other links to Spanish translations. The Spanish translation version is sold in a bundle with the official English version. These belong in any room where vaccinations are given.
