A green, four-fold lifetime immunization record card. The card has spaces for all U.S. recommended vaccinations for infants, children, teens, and adults.

It's printed on rip-proof, smudge-proof, water-proof paper, and is designed to be used from birth through adulthood.

The card is credit card size, measuring 3-1/2" x 2-1/8" when folded, so it will easily fit in a wallet.
